APPLICATION PROCEDURE : -

• Applications must be made using the prescribed application form, which can be obtained from the Public Market Division, Health Department, at the Jalan Meru Central Market Complex Branch Office, 41050 Klang, at a cost of RM1.00, with an additional RM10.00 processing fee.
• Completed application forms must be submitted to the above address for processing.

GUIDELINES FOR PREMISES APPLICATIONS :-

• Only applicants who have fully completed the Premises Application Form will be called for an interview.
• Incomplete applications WILL NOT BE CONSIDERED.
• Applicants must be MALAYSIAN CITIZENS and born in Malaysia. Applicants who were NOT BORN IN MALAYSIA are required to submit a copy of their BIRTH CERTIFICATE.
• Applicants must be at least 18 years old and not more than 60 years old, based on their DATE OF BIRTH.
• Applications for locations where there are NO VACANCIES / NOT ADVERTISED will be REJECTED without an interview.
